Full payment is due at the time of service. We do require a credit card on file for all initial and existing patients which is utilized only for patient responsibility as dictated by the service fee charged (self-pay) and our cancellation policy.
We understand schedules can change with short notice, however, we do request that you call to cancel or reschedule your appointment at least 24-hours in advance. This gives us the opportunity to offer the appointment to another person in need.
All appointments missed or cancelled in less than 24 hours will be subject to cancellation fee which is $200 for new appointments or $80 for follow-up appointments; and will be charged to the card on file.
