What Is a Psychiatric Assessment?

What Is a Clinical Assessment at CHPS?

Our clinical assessment is a comprehensive evaluation conducted by a psychiatrist or psychiatric nurse practitioner to accurately diagnose mental health conditions and establish a stable, personalized foundation for care.

Unlike a general intake appointment, our assessments are collaborative - conducted in partnership with a clinical pharmacist and pharmacotherapy expert to ensure that any medications considered are safe, evidence-based, and tailored to your individual biology and health history.

Expert Diagnosis & Treatment Planning
A psychiatric assessment is the first and most important step toward effective mental health care. Our expert team works with you to understand the full picture of your health and build a treatment plan designed around you.

What to Expect?

A thorough, whole-person evaluation

During your assessment, you'll meet with a psychiatrist or psychiatric nurse practitioner who will:

-Review your mental health history and current symptoms

-Assess how your physical health may be contributing to your mental well-being

-Collaborate in real time with a pharmacotherapy expert to inform any medication considerations

-Provide a formal diagnosis and a clear explanation of your condition

-Co-develop a personalized, integrative treatment plan

Our Approach

We practice evidence-based, integrative care which means we draw on the most current clinical research while treating you as a whole person, not just a set of symptoms.

Every assessment is:

Individualized:  tailored to your unique history, needs, and goals

Collaborative: informed by a multidisciplinary team

Coordinated: your mental and physical health are addressed together

Compassionate:  delivered in a safe, non-judgmental environment
